Consent for personal data and/or photographs and/or footage
By ticking the consent box I consent to The Memory Shed using personal data, details, photographs and/or footage (audio and video) including images of me and/or of my child, to create a personalised storybook. The personal data, details, images and footage could be used in print and digital media formats including print publications.
I understand that in some circumstances, especially in relation to digital media, images can be viewed throughout the world and not just in the United Kingdom and that some overseas countries may not provide the same level of protection to the rights of individuals as UK legislation provides.
I have read and understood these conditions and consent to all the information, images and data being used as described. By signing this consent, I further confirm that I consent to the use of my child’s images and data, if relevant, and that consent from another party is not necessary on behalf of my child.
The Memory Shed is committed to processing personal data in accordance with the UK General Data Protection Regulation (UKGDPR). The personal data collected on this form will be held securely. Your rights
You have the right to request to see a copy of the information we hold about you and, if applicable, that of your child, and to request corrections or deletions of the information that is no longer required. You can ask us to stop storing your personal data, details, images or footage at any time, in which case we will delete it but it may continue to appear in anything already in circulation whether in print form or digitally. The information you provide us with is not used in a commercial sense and shall only be used to create a personalised storybook about the person that appears in the book we create. We are therefore unable to remove any personal data or information you provide to us from the storybook in question and you cannot withdraw your consent specifically in relation to this.
